INDIANAPOLIS — Community Health Network, in partnership with Anthem Blue Cross and Blue Shield, has announced plans to assist the uninsured with enrollment in the Indiana Health Insurance Marketplace. Under an agreement with Anthem, Community Health Network physicians, hospitals and facilities will serve as the primary provider network for Central Indiana residents who purchase an Anthem health benefits plan on the Indiana Health Insurance Marketplace, or “exchange.”
To assist patients and health care consumers, Community is offering enrollment events through the remainder of the year at the following locations:
• Community Hospital East, main lobby: Oct. 22, Nov. 19, Dec. 10
• Community Hospital North, main lobby: Oct. 30, Nov. 21, Dec. 12
• Community Hospital South, main lobby: Nov. 12, Dec. 3
All events will begin at 4:00 p.m. and end at 7:00 p.m.
Those who want to enroll for coverage during one of these sessions should bring the following: social security numbers (or document numbers for legal immigrants); employer and income information for every member of the household who needs coverage (pay stubs or W-2 forms—wage and tax statements); policy numbers of any current health insurance plans covering members of the household; and a completed Employer Coverage Tool for every job-based plan members of a household are eligible for (available on eCommunity.com/enroll).
The deadline for consumers to enroll in the Marketplace is December 15, for coverage beginning January 1, 2014. The final deadline to enroll is March 31, 2014. For more information on the Indiana Health Insurance Marketplace, visit: eCommunity.com/enroll, or call (317) 883-5680.
